First Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Typically, treatment can be completed the same day as the consultation. However, more complex cases may require additional appointments to complete the treatment properly.

Please assist us by providing the following information at the time of your consultation:

Your referral slip.

 If you have medical or dental insurance that requires paper forms to be filled out please bring them with you.  

We encourage our patients to register online and provide their information prior to your scheduled office visit.  (Click on the patient registration link located on the right hand side of this screen.)  This greatly reduces in-office wait time.  Please be assured that your information is transmitted with SSL encryption and your data will remian secure with us.  Our office maintains strict HIPAA confidentiality and will not share your information with any third party not permitted by you.

Please alert the doctor upon your arrival if you have a medical condition that may be of concern prior to surgery.
